PLAYERS WILL NOT BE OFFICIALLY REGISTERED UNTIL ALL REQUIRED PAPERWORK IS SUBMITTED AND REGISTRATION FEES ARE PAID.

The Simi Valley Patriots football program is divided by age and weight into FIVE divisions:

Pee Wee
 7-8
 105 lbs.
Bantam
 9-10
11		 125 lbs.
75 lbs. (+5 playoff pounds)
Midget
 11
12		 140 lbs.
95 lbs. (+5 playoff pounds)
Junior
 12
13		 155 lbs.
100 lbs. (+5 playoff pounds)
Senior
 13-14 190 lbs.

  • In addition to completing the form, you must also provide a photo (no larger than 2"X 3 ¾ ").

  • You can use a school picture, sport photo, or other.

  • Please attach photo to the form in the box provided.

  • Photo will not be returned


    **PHYSICAL EXAM FORM
  • Any physical exam dated in 2008 is acceptable.

  • THIS FORM MUST BE PROPERLY COMPLETED BY YOUR PLAYER'S PHYSICIAN AND SUBMITTED ON OR PRIOR TO THE FIRST DAY OF PRACTICE.

  • PLAYER CANNOT PRACTICE WITH THE TEAM UNTIL THIS FORM HAS BEEN RECEIVED BY THE TEAM MANAGER/COACH.


    BIRTH CERTIFICATE
  • Original Birth Certificate

  • Passport will suffice

  • Hospital certificates will not be accepted

  • Returning P.Y.F.L. players do not need birth certificate.
